📄 Application for Academic Records
Use this form to request certified copies of your academic records, transcripts, or related documents from the Faculty of Agriculture.
Submission Instructions
- Download and print the relevant form.
- Complete all sections legibly in block letters or type-fill the form.
- Attach any required supporting documents as specified in the form.
- Submit the completed form to the Faculty Office (Senior Assistant Registrar) in person or by post.
- Pay the applicable fee at the Finance Division, University of Ruhuna, if required.
